1.     Description of the clinical problem/question, specifying the population involved, locations or exposures that may influence the occurrence or outcome of the problem, clinical settings where nurses may provide care to patients with the problem, population prevalence of the problem, and why this problem is important.  Possible reasons for its importance include (but are not limited to) it affects a large number of people in the population (even if the outcomes are likely to be good a whole lot of people are affected), it can cause devastating damage to those who get it (this is important even for rare conditions), it costs a lot of money to treat, it affects parents who have responsibility for young children and/or teens, it is highly contagious and can spread rapidly in communities, or perhaps there is another reason it is important.  But consider human lifestyle effects, cost effects, and things like that.  Then discuss consequences for individuals, families, and communities if the problem isn’t adequately addressed. The selected problem is nurse retention. Accordingly, nurse turnover remains a significant issue that affects the entire healthcare sector. In the study by Lockhart, L. (2020), the lowest nurse turnover rate leads to financial and quality loss, while the highest rate causes major consequences for an organization. Hospitals are struggling in trying to retain experienced and novice nurses. According to Lockhart (2020), 18% of new nurses will likely change their jobs or professions the first year after graduation. After two years, a third will leave as well (Lockhart, 2020). Nursing turnover averages 19.1% nationwide and is expected to increase, with a vacancy rate of 8% (Lockhart, 2020). Nurse turnover causes a situation of nurse shortage in a health facility. Subsequently, this creates a situation characterized by increased strain on the remaining nurses, which leads to stress and burnout that eventually impacts the overall quality of care given to the patients. Most of the problems in most hospitals could be resolved if the problem of nurse turnover is addressed.

Similarly, the study by Hu et al. (2022) stipulated that 35-60% of nurse graduates were likely to change employment within the first year. Consequently, this could cause a shortage of clinical nurses and lower overall quality of care in a health facility. Other consequences include increasing costs of recruiting employees. A low turnover rate is largely linked to safe and positive patient outcomes. Most causes of turnover among nurse graduates who have acquired employment as nurses are lack of clarity of nursing roles by an organization, poor communication with management on critical issues, lack of recognition for nurse performance, and stress and burnout (Dewanto & Wardhani, 2018). All these factors can be addressed within an organization to ensure that the environment is conducive to retaining nurses who have graduated and are qualified to hold different positions in a health facility.
