  • Promoting mental health is very important to achieving positive health outcomes. Achieving healthy mental status requires the collaboration of PMHNP and other interprofessional teams. This is because a PMHNP is a professional providing mental health therapy. Therefore, as an RN who is not in an advanced practice role, it is essential to collaborate with a PMHNP regarding health promotion, preventive medicine, health protection, anticipatory guidance, and counselling. This is because the RN practice areas involve providing coordinated patient care that focuses on diagnosis and disease management. However, as a PMHNP, the practice area deals with patient-centred mental health care, which involves health promotion through the prevention of mental health issues, prevention, guidance, and counselling. Collaborating with a PMHNP ensures that proper patient care is being provided. This is because a PMHNP understands the critical components that promote mental health, including physical activities, support, community, self-care and environmental health.
  • Staying up to date with those areas of primary care when the primary focus is psychiatry may involve joining professional organizations, collaborating with other interprofessional and advancing in education. Almost every nursing specialty has a professional organization, and joining the organization can help remain up-to-date with changes and trends in any practice area. Through the organization membership, one gets full access to resources, conferences and events to learn more about some areas of psychiatry. Secondly, collaborating with other skilled experts may help keep up-to-date with those areas. According to Rao et al., (2015) collaborating with interprofessionals provide cross-education which enhance patient care thus reducing morbidity and mortality rates.   • ethnic identity: The community where I plan to work is Kent County, Delaware. In terms of ethnic identity, the society is multicultural. Ethnic identity is a person’s social belonging to a specific ethnic group. The multicultural community comprises a diverse population with variances in religion, race, culture, socioeconomic status, ethnicity, and age.
  • socioeconomic conditions: The socioeconomic conditions of the community are varied. This means different people have different socio-status where one is either ranked as of high, middle, or low socioeconomic group.

 

  • emotional and spiritual needs: The Kent community also has varied spiritual needs, as each individual has unique physical and emotional needs.

 

  • literacy levels: The literacy level of the community is also various. The literacy level is mixed as individuals have different educational backgrounds; where others have attained degree programs, and others have only achieved a high school certificate.   • values and preferences.  Lastly, the Kent Ware, Delaware, community’s values and preferences are also varied. This means that different individuals have unique understandings, concerns, and expectations about their health outcomes.

 

  • What resources are available to meet those needs?  The available resources that can help meet the community’s identified needs include mental health centers, community and private practice, and acute crisis care. The availability of a community mental health center plays a significant role in meeting the community’s diverse needs. Through the facility, community members can access adequate mental health education. In addition, the facilities may establish health promotion programs. The facilities provide self-management education, help members adopt and maintain healthy behaviors and create a physical and social environment that effectively enhances mental health.

 

  • What gaps in mental health resources have you identified?

The resource gap I have identified is inadequate practitioners. The lack of human resources provides a shortage of community healthcare services, which means the community’s needs may not be addressed (Carbonell et al., 2020). Mental health professionals usually have challenges enhancing patient adherence to medications and therapy regimens. Medications and therapy are essential to overall care and health outcomes. While pharmacological treatment is considered the first treatment for mental disorders, therapeutic alliances between a patient and provider effectively promote health. A therapeutic approach may involve sharing situations of a patient who benefitted from a mental health promotion or support program (Bombard et al., 2018). While sharing a successful patient situation, maintaining the confidentiality of patient information is integrated into the psychology code of ethics. In such a scenario, maintaining the confidentiality of patient identity also protects patient information. Psychiatrists understand the legal implications of breaching patient privacy and confidentiality.

In my practice, I had a situation where a patient did not want to involve her husband in family therapy. I remembered an encounter with a similar patient, and I decided to share to promote mental health promotion. The patient was having trouble in her marriage because she had suffered from trauma from multiple cases of abuse by her uncle when she was a little girl. She had not told her husband, and this was breaking her marriage. She was finally ready to open up but did not want her husband involved in the therapy. However, after guidance from the counselling sessions on why it was essential to involve the husband, the patient agreed, and a family therapy appointment was set. On the first appointment, the patient’s husband was updated, and to her surprise, her husband offered her support, attended all sessions, and they have enjoyed their marriage ever since.
