When researching nursing theorists, it is difficult to just choose one that has shifted the paradigm of nursing practice. However, a theorist that captivated me particularly is Lydia E. Hall. She claims responsibility for developing the Care, Core, Cure theory, better known as the “Three Cs of Lydia Hall.” (Gonzalo, 2019) This theory enthralled me as a prospective PMHNP student, as it outlines the importance of the contribution of developing interpersonal relationships with the individual to the next step in the theories process.

“Hall defined Nursing as the “participation in care, core and cure aspects of patient care, where CARE is the sole function of nurses, whereas the CORE and CURE are shared with other members of the health team.” (Gonzalo, 2019) Each health care team member fulfills their role by contributing to the individuals care plan, allowing the nurse to provide skilled services and nurture thriving patient outcomes. With the care model successfully implemented, the development of the CORE process is facilitated by nursing care being provided. Then followed by the CURE aspect, which includes medications and treatments. This theory is an integrated approach to patient care that provides a realistic model of how the health care team must work together to support the individual in need. This begins with the nurse’s development of relationship with the individual and then completed with patient centered interventions, the nurse actively advocating for the individual throughout the process.

Hildegard Peplau, psychiatric nurse and theorist, left an unforgettable stamp on the nursing profession including the lives of the mentally ill (Barbara J. Callaway, 2002). Peplau developed the Theory of Interpersonal Relations in the mid-20th century, which to date still remains highly influential in the psychiatric nursing profession (Arabacı, 2019). This paradigm emphasizes the therapeutic communication between the nurse and patient, a key aspect of psychiatric nursing. The theory consists of four components: Orientation Phase, Identification Phase, Exploitation Phase, and Resolution Phase (Barbara J. Callaway, 2002). These stages focus on establishing rapport, building trust, developing coping skills, recognizing patient requirements, and assisting the patient in achieving goals for transitioning to discharge and continued treatment. Utilizing a specific theoretical framework, such as Peplau’s, is essential for enhancing the quality of nursing care (Arabic, 2019)
Adult Psychiatric Care
Peplau’s theory intrigues me as I relate to the four components that define the theory, as well as integrating and implementing the components in my daily patient centered care approach. Personally speaking, the Orientation Phase of Peplau’s theory is undoubtedly the most enjoyable aspect of patient care in my nursing profession. This stage of the theory involves establishing rapport and trust, identifying the patient’s needs and concerns, all while helping the patient feel comfortable and secure. Establishing rapport is essential, particularly with psychiatric patients, it promotes effective communication, instills trust, improves patient care, and demonstrates empathy (Mersha et al., 2023). During this stage I sit with patients one on one, listen to their concerns, validate their feelings and fears, and provide empathy and compassion. Validating patients’ feelings and emotions can help them feel understood and supported. Allowing the patient to open up and express their feelings promotes a positive therapeutic relationship. The orientation phase of the theory is particularly important as the level of trust and rapport that is established will aid in reducing anxiety in the patient. After establishing rapport, the three remaining components of the theory typically align effortlessly.
Knowing that I can positively influence a patient’s care is extremely important to me. It motivates me to consistently deliver exceptional patient-centered care while advocating and empathizing with each individual patient. Being a psychiatric nurse presents both challenges and rewards, and for many nurses, it is a deeply meaningful and fulfilling career (Barbara J. Callaway, 2002). It is my hope that I can continue to promote and enhance patient satisfaction and improve health outcomes. Implementing Peplau’s Theory of Interpersonal Relations provides the framework to continue to provide the utmost care to each individual patient. I found Peplau’s Theory to be the most relatable study in my career thus far and thoroughly enjoyed learning about it.
