Topic: medication error during a patient’s transition of care
The goal of this paper is to apply the principles of data collection to a problem or opportunity for improvement in your workplace (your immersion project topic). This requires computer skills with Excel Software and knowledge of structural, process and outcome measures.
Guidelines
● In Excel, create a spreadsheet that you would use to collect data related to your topic.
● Include mock data in the spreadsheet and create a graph using this data. Ensure that your graph includes a title and clear labels on both axes.
● In a Word document:
○ Explain how the data is relevant to your topic.
○ The type of measures (structural, process or outcome).
○ How this data would be collected.
○ Lastly, offer your interpretation of the mock data.
○ Copy and paste your data table and graph into the Word document with your written analysis.
● APA Format with a title page. Maximum 2 pages in length (excluding title page, data table and graphs pasted into the document.
● Review the rubric prior to submission.
